Register with the Surgery

Register with the Surgery – please view our Practice leaflet for information about the Practice.

Everyone in the UK has the right to free healthcare from a GP. We ask you to show some form of ID to prove who you are. If you don’t have any proof of ID, you can still register and provide ID at a later date. Immigration status or nationality does not matter and you will not have to share immigration documents.

Registering with the Surgery

To register with the surgery:

· Ask for a ‘Registration Form’ from Reception. Our quieter times are Tuesday – Thursday, 14:00-18:00

NHS Number

Patients that were born in the UK should have been allocated an NHS number. You can search for your NHS number here: find your NHS number.

Current Medication 

Is it very useful if you can get a list of your current medications from your previous GP or your pharmacist so we can continue to issue your prescriptions.

Named GP

An accountable GP will be assigned to every patient when they register with St Luke’s Surgery. This will be your ‘allocated GP’ but you can book an appointment with any of the Clinicians at the practice. Where a preference is expressed reasonable efforts will be made to accommodate this.

Health Checks

All patients over the age of 16 will be offered a NEW PATIENT HEALTH CHECK.

Those aged between 40-74 years old will be offered an NHS HEALTH CHECK.

Over 75s will be offered an ANNUAL HEALTH CHECK.

Home Visits are available for housebound patients only.

Temporary Registration

If you are in the area and you need medical assistance please contact the practice. We can register you as a temporary patient and offer you an appointment.

Information about you visit will be sent to your regular GP to update your medical records.

Accessing someone else’s information

If you are a Parent or you Care for someone, you might be able to get access to someone else’s online medical record. This is useful if you book appointments or request prescriptions on the behalf of someone else.

Once proxy access is set up, you can access the other person’s profile in your NHS account, using the NHS App or website. The NHS website has information about using linked profiles to access services for someone else.

If you would like proxy access please speak to a Receptionist.

Patients Rights and Responsibilities

At the practice your health is our utmost priority and in partnership with you we will do our best to look after your health.

Patients have a right to:

  • Be registered and allocated a named General Practitioner
  • Change doctor if desired
  • Be offered a New Patient Health Check on joining the Practice
  • Receive appropriate drugs and medicines
  • Be referred to a specialist or second opinion if they and the GP agrees, patients can choose which hospital they would like to be referred to
  • Be treated courteously by all staff at the practice
  • Be given sufficient information about their condition to make an informed choice about investigation and treatment.
  • Be offered a home visit, if living within the practices catchment area and currently housebound
  • Have the right to review their medical records; subject to the relevant Acts.
  • Confidentiality— Please ask for ‘How we use your information Leaflet’, If you do not wish to share you medical record, please inform a Receptionist.
  • Be treated without discrimination.

Patients responsibilities are:

  • To keep booked appointments or cancel in good time
  • To know that one appointment is for one person and one problem only. If you have multiple problems, please inform a Receptionist and a longer appointment can be made. (Normal appointment duration is ten minutes).
  • To give a brief reason for your appointment so we can book an appointment with the most appropriate clinician for your needs.
  • To speak to your local chemist when suffering from a minor ailment. Antibiotics are available at chemists for certain illnesses. Please note antibiotics will not help viral infections.
  • To use NHS services appropriately – If you have spoken to your local chemist and this has not helped we can book you an appointment at the practice. The Accident & Emergency Department should only be used in medical emergencies.
  • To treat staff with the same respect that you expect to receive from the practice. We operate a Zero Tolerance Policy against abusive behavior and you will be removed from the practice list if you act inappropriately.
  • Letting us know when you change your address or telephone number
  • Only requesting a home visit when it is truly necessary for medical reasons– when a patient is too ill or immobile to come to surgery.
  • Use the out of hours service if your symptoms cannot wait till we re-open.
  • Observing the no smoking policy around the building and car park.
