Appointments available at St Luke’s Surgery:

If your medical issue is not a 999 emergency you can request an appointment at St Luke’s Surgery. We have online, face to face, video, telephone and home visit appointments available. Our Receptionists have been trained to take a brief description of your medical issue so they can then book you an appointment with the most appropriate clinician for your medical need.

PLEASE USE ONE OF THE OPTIONS BELOW TO BOOK AN APPOINTMENT: 

1) ONLINE TRIAGE FORM  – (ONLY USE THIS OPTION FOR NON-URGENT APPOINTMENTS, IN AN EMERGENCY PLEASE DIAL 999 OR FOR URGENT SAME DAY APPOINTMENTS PLEASE PHONE 01922 775136) This is a quick and easy online form where you can give a brief description of your medical problem, state how long you have experienced this medical problem and what treatment/s you have already tried. You can also attach photos (if necessary). Once the practice has your request they will respond to you that same day.  Please remember the practice is closed on weekends and bank holidays, if you need to speak to someone urgently please call Ournet on 01922 501999 or NHS 111.

3) WALK IN TO THE PRACTICE – and speak with a receptionist face to face between 8am – 6:30pm, Monday – Friday.

4) DOWNLOAD THE NHS APP – Here you can book several appointments, request repeat prescriptions, view your medical record, gain information about your health and register as an organ donor.

Please tell us:

· if there’s a specific doctor, nurse or other health professional you would prefer to respond

· if you would prefer to consult with the doctor or nurse by phone, face-to-face, by video call or by text or email

· if you need an interpreter

· if you have any other access or communication needs

Home visits

Home visits will only be provided for patients that cannot leave their home due to medical conditions of severe frailty.

To request a home visit, it’s helpful if you phone the practice as soon as possible, home visits are usually done after 12pm.

Cancelling or Changing your Appointment:

To cancel your appointment:

· use your NHS account (through the NHS website or NHS App). The NHS App can also be used to request repeat prescriptions, view your medical record, show vaccine status, to gain a COVID Pass, to gain information about your health and to register as an organ donor.
