Urgent Appointments

To request an urgent appointment for today or tomorrow (Monday to Friday):

· Contact your practice using an Online Form – Complete a quick and easy online form and attach photos (if necessary) Online Triage Request. A member of staff will review your request and will contact you within 2 working days. Please remember the practice is closed on weekends and bank holidays.

· Phone the practice –  on 01922 775136, Monday – Thursday 8am – 6:30pm and Fridays 8am – 1pm (The phone line is closed every second last Wednesday of the months for staff training). The Receptionist will take a brief reason for your appointment, all Receptionists have been trained to triage patients so they can signpost you to the most appropriate Clinician or service. We have several appointments available and various methods such as telephone, video, SMS and face to face appointments available.

· Visit the surgery  – and speak with a receptionist, Monday to Thursday from 8am to 6:30pm, Fridays 8am – 1pm.

Routine appointments or Simple Questions

To request a routine appointment or to ask a simple question:

· Use your NHS account (through the NHS website or NHS App) to book a screening test or vaccination – Link to NHS APP Click Here. The NHS App can also be used to request repeat prescriptions, view your medical record, show vaccine status, to gain a COVID Pass, to gain information about your health and to register as an organ donor.

When you get in touch, we’ll ask what you need help with.

We will use the information you give us to choose the most suitable doctor, nurse or health professional to help you.

Extended GP Access

GP appointments are available at Darlaston Health Centre, Pinfold Health Centre, Broadway Medical Practice and Portland Medical Practice during the following times:

Weekday evenings: 6:30pm -9pm

Saturdays: 9am – 5pm

There are GP, cervical screening and phlebotomy appointments available at OurNet. For phlebotomy appointments, ensure that you have requested the bloods on your system, at practice level, and then OurNet will print the labels.

Your appointment

We have several healthcare staff at the practice, social prescribers, mental health nurse, physiotherapist, pharmacist etc. We can offer you a consultation:

· by phone

· face to face at the surgery

· on a video call

· by text or email

Appointments by phone, video call or by text or email can be more flexible and often means you get help sooner.

Cancelling or changing an appointment

To cancel your appointment:

· use your NHS account (through the NHS website or NHS App). The NHS App can also be used to request repeat prescriptions, view your medical record, show vaccine status, to gain a COVID Pass, to gain information about your health and to register as an organ donor.

Out of Hours – When we are closed

If you need help when we are closed If you need medical help now, use NHS 111 online or call 111. NHS 111 online is for people aged 5 and over. Call 111 if you need help for a child under 5.

Call 999 in a medical or mental health emergency. This is when someone is seriously ill or injured and their life is at risk.

Extended GP Access

Local Chemist

If you need help with your appointment

Please tell us:

· if there’s a specific doctor, nurse or other health professional you would prefer to respond

· if you would prefer to consult with the doctor or nurse by phone, face-to-face, by video call or by text or email

· if you need an interpreter

· if you have any other access or communication needs

Home visits

If you are housebound and need an appointment, we will do a home visit. We will phone you first to understand what you need.

To request a home visit, it’s helpful if you phone the practice as soon after 8am as possible.
